Engine Performance problem

Hi can anyone help me find whats causing my car to shake really bad while accelerating and not accelerating. I went to a shop they said one spark plug boot was bad so they replaced it but a couple of days later it was back to the same. When i drive it sounds like the engine is really tired and stop at a light it shakes where my body is shaking with the car. But i found out it shake less then it does if i change it to neutral on a stop. Also i havent changed my oil in a while. Is this whats causing the problem? The engine light blinks when ever i get to 40mph and it just lights up below that. It can bearly go over 40mph and somtimes if im lucky it switch back to normal and the car is fine again. I would really appreciated if someone give me some tip on it.
