Rpms increase and decrease while driving

My vehicle will increase rpm's to 4000 then decrease. When it goes up again, it clunks. Speed remains pretty much the same.

Does this happen with the engine in neutral or while you are moving?

while moving. noticeable when driving in the highway, fifty to sixty mph.

Okay, that sounds like the transmission is not shifting from a lower gear.

Does it have any codes set?
